Preventing Medical Errors

Medical errors are what happen when some part of a client’s treatment plan doesn’t work out as expected—or when the wrong plan was used in the first place. Medical errors happen because:

  • Something is always “new and improved,” such as technology, medical equipment, treatments and medications.
  • The healthcare system has flaws, like short staffing.
  • Healthcare workers are only human!

PREVENTING SURGICAL MISTAKES

  • Avoid washing off any markings that identify a surgical site.
  • Let your supervisor know if you see a client eating or drinking while under a “nothing by mouth” order. 
  • Report a loose, wet or smelly surgical dressing right away.

PREVENTING MEDICATION ERRORS

  • Report when a client seems confused about his/her medications. 
  • Teach clients to stop anyone who tried to give them a medication without checking their identity first. 

PREVENTING INJURIES

  • Encourage clients to shift position every 15 minutes to avoid pressure ulcers. 
  • Know the fall “history” of each of your clients to help prevent future falls. 
  • Keep your clients’ environment organized and clutter-free. 

TEAMWORK PREVENTS ERRORS

  • Have a basic understanding of your co-workers’ jobs so you can all work together. 
  • Be sure you are clear about what your supervisor expects from you.  
  • If you have questions about your assignment, don’t be shy.  Ask! 
  • Don’t ever perform a task that is beyond your training—no matter who asks you to do it!
  • Remember to tell physical and occupational therapists how your clients are getting along in between therapy sessions.
